PPO Insurance Rehab Placement — New Jersey
Why PPO Plans Are Ideal for Inpatient Placement
PPO (Preferred Provider Organization) plans offer both in-network and out-of-network benefits. Out-of-network coverage is what gives you access to a wide range of inpatient programs — including many top-tier residential facilities that don't contract as in-network with every carrier. Most commercial PPO plans cover inpatient rehab with deductibles and coinsurance similar to any other inpatient hospital stay.
Which PPO Carriers Our Advisors Verify
Horizon Blue Cross Blue Shield of New Jersey, AmeriHealth New Jersey, Aetna, Cigna, UnitedHealthcare, Oxford (a UHC subsidiary), Anthem (for out-of-state PPO), and most Blue Cross Blue Shield plans from other states with out-of-network benefits. Also most self-funded ERISA PPO plans administered by these same carriers.
What Verification Tells You
In-network and out-of-network deductibles, remaining deductible YTD, inpatient per-admission or per-day cost-share, coinsurance percentages, out-of-pocket maximum, preauthorization requirements, whether detox and residential are authorized separately, and whether a mental-health-carve-out vendor handles the review. Our advisors run this verification in 10–20 minutes and walk you through the numbers.
Preauthorization Under NJ Parity Law
Under A2031, any SUD utilization review must use ASAM criteria. That means denials must be justified by ASAM documentation, not by internal carrier criteria. If a plan denies preauthorization on flimsy grounds, the denial is appealable through NJ DOBI Parity Appeals at (609) 777-9470. Inpatient programs in our network routinely handle these appeals.

Frequently Asked Questions
Do I need an in-network provider?
No. PPO out-of-network benefits give you access to essentially any licensed inpatient program willing to bill your insurance. Many top residential programs operate on an out-of-network basis with major PPO carriers.
What's the difference between PPO and HMO for rehab?
HMO plans typically require in-network providers and a referral from a PCP. PPO plans don't require either. For inpatient rehab placement, PPO coverage is substantially easier to work with.
What if I have an HDHP with HSA?
High-deductible PPO plans still provide out-of-network benefits; you'll typically meet your deductible early in an inpatient stay and the plan picks up the coinsurance share from there. HSA funds can be used toward the deductible and cost-share.
